Home Hospice Aide
Shift: Day or Mid shift, 5/8's per week, every other weekend.
Minimal requirements: •MUST BE A STATE TESTED NURSING AIDE IN OHIO (STNA) no exceptions.
2+ years of home hospice experience
High school graduate or GED
Reliable transportation to the patients, families and communities we serve, we are the epicenter of compassion and care, wellness and prevention. Driving is an essential function of the job so reliable transportation is a must. Assists patients in cleanliness and hygiene: bathes select patients by giving complete/partial baths, assists with tub baths, showers, perineal care and skin care, assists and instructs patients/families in oral hygiene and denture care; performs Foley catheter care. Documents care in home care record according to department standards. Performs other patient care services; positions patients, performs passive ROM exercises, ambulates patients, applies hot-cold packs; applies non-sterile dressings, assists patient/family with colostomy care, etc. Takes vital signs. Must have ability to move/lift transfer bed-bound or physically impaired patients. Helps patient achieve quality of life through providing companionship and emotional support, providing reassurance, listening and encouraging social contact. Reports significant physical/mental patient changes to RN and provides care in conjunction with team plan of care. Seeks assistance as needed. Travels to patient home, as needed. Picks up supplies/medications for delivery, calling patient's family with estimated time of arrival, documentation, etc. Census of 60, with an average of 6 daily visits.
